JOB FUNCTION:
One day per week, the NP (Indigenous Health) will provide culturally safe and comprehensive care for individuals seeking support from UHN’s SMP with an emphasis on primary care and the social determinants of health. The remaining 4 days they will work onsite at Auduzhe Mino Nesewinong to provide care for individuals and families seeking support. The NP (Indigenous Health) will use advanced Nursing knowledge, skill, and judgment to assess and manage clients who may present with complex social and health conditions such as unmanaged chronic disease, mental health, substance use, sexual health, and the ongoing effects of colonialism. The NP (Indigenous Health) works collaboratively with the interprofessional team to provide outreach and on-site episodic, transitional, primary care, and public health services to clients respecting PHIPA and Indigenous privacy. The Nurse Practitioner (Indigenous Health) will be expected to perform cross-functional and/or other duties consistent with the job classification, as assigned or requested
